Waiver Forms for Unaccompanied Minors
If a participant is under 18 and will not be accompanied by their legal parent or guardian to the park, they'll need to bring a signed release form.
These waivers must be printed and completed before arriving for check-in.
For participants who are 18 or older or minors accompanied by their parent/guardian, no action is needed in advance — waivers will be filled out at the park.
Each location has its own waiver, so please download the correct one based on where your tour is booked.
ADVENTUREWORKS programs involve a variety of events that often include traditional classroom settings, warm-up games, group problem-solving, high and low challenge courses, and occasional rigorous physical activity.
The level of participation in an ADVENTUREWORKS activity is at all times completely voluntary and up to the individual's choice. Yet despite our strong emphasis on safety, there is always an inherent risk which must be assumed by each participant, that he or she may suffer an emotional or physical injury or disability in the course of any of our programming. Certain health/medical information must be made known to the instructor(s) conducting programs so that they are prepared to respond appropriately if the need arises.
Anyone under age 18 will need to have a signed release form if they're not accompanied by a legal guardian.
